Celebrate the Life of Michael Mazzone, MD

It is with a heavy heart we share the passing of Mike Mazzone, MD. Mike was a member of WAFP for 30 years and on the board from 2018-2021. He was a member of the Waukesha Family Medicine Residency Program for 23 years and spent 19 years as the Program Director.
 
While doing what he loved, he was able to create the WAFP Passport Program which brought more medical students to visit residency program in Wisconsin, created the WAFP residency directors committee, and helped create and present demonstrations to medical students as how to interview with residency programs, contributed to the Consider Wisconsin book, testified on behalf of WAFP at the State Senate and House Committees on various family medicine issues, and so much more.

Awards and Honors Include:

  • Meritorious Service Medal for work on Emergency Room Reorganization
  • Air Combat Command Certificate of Recognition for work on Emergency Services Transitions Team
  • Department of Family and Community Medicine Clinical Teaching Award, Medical College of Wisconsin
  • Best Doctors of America
  • Department of Family and Community Medicine Educational Project of the Year Award (2004)
  • Educational Project of the Year Award (2006)
  • Medical College of Wisconsin Faculty Vitality Award
  • Association of Family Medicine Residency Directors Program Director Recognition Award Bronze Level
  • Department of Family and Community Medicine Educational Project of the Year Award (2010) P4 project
  • Department of Family and Community Medicine Published Paper of the Year Award (2010) Majors and Masteries Curriculum
  • Waukesha Family Medicine Residency, Faculty of the Year
  • Office of the Secretary of Defense, Patriot Award for contributing to National Security and Protecting Liberty by Supporting Employee Participation in America’s National Guard
