The PCMH Network offers WAFP members opportunities to contact and network with other member physicians who have implemented PCMH components. The Network is for sharing information and should not be construed as professional consultation.
The PCMH Network is available on the understanding that users exercise their own judgment and consideration with respect to its use:
- PCMH Network contacts are sharing information on a voluntary basis. Please be respectful of time. If you move to phone contact after email correspondence, try to limit conversations to 10-15 minutes.
- Respect the willingness of the volunteer to share or not share specific information.
- Please do not request site visits.
- The contact information should not be used for the purpose of making sales contacts; promoting products, services and events; conducting surveys or soliciting for donations.
