What is the Wisconsin Academy of Family Physicians doing?

Advancing the Patient Centered Medical Home (PCMH) concept is at the top of the list of priorities for the Wisconsin Academy of Family Physicians.

We’re making it job #1 because we believe PCMH is our best hope for meaningful health care reform.  It’s an approach to healthcare delivery that improves health outcomes, enhances access and reduces overall costs by building an effective partnership between family physicians and their patients. 

As a result, for the next 3-5 years, our goals are to:

  • Help our members understand, embrace and implement PCMH
  • Educate payers and legislators about the benefits of the medical home concept and achieve payment reform.

HERE'S WHAT WE'RE DOING

For our WAFP Members:

  • We’ve compiled tools you can use to better understand PCMH and its benefits to your patients and your practice.  Many of the documents are available for download on the right side of this web site page. 
  • We’ll begin soon to conduct monthly phone/web conferences, open to all members, staff and colleagues.  Each conference will feature a PCMH topic and will be followed by Q&A.

On the Policy Front:

  • Our lobbyist is actively talking with Wisconsin state legislators about the benefits of PCMH.
  • In November, 2008, we testified before the Wisconsin Legislative Council in favor of a PCMH pilot program in the state. Download the testimony.
  • Developed a proposed PCMH Medicaid Payment Program. Download a program summary.

Here’s what you can do:

  • Discuss the concept with your peers and within your clinic. (WAFP will also make itself available to present the PCMH story in person to you and your colleagues, if requested.)
  • Take the MHIQ and compare your answers to the NCQA PPCPCMH™ Standards — a national recognition program based on the Patient Centered Medical Home.
  • Develop a workplan to become a Patient Centered Medical Home.
  • Look at the Additional Resources to the right and share them with others in the practice.
