Wisconsin Academy of Family Physicians
seeks nominations for

Family Medicine Educator of the Year

Each year the Wisconsin Academy awards one family physician the title of "Family Medicine Educator of the Year" to recognize a physician who exemplifies the tradition of the physician as teacher.

Nominees are evaluated on the following criteria:

  • Recognized by peers and students for excellence in teaching, and is compassionate to patients as well as to those he/she is teaching
  • Is directly and effectively involved in community affairs and in activities that enhance the quality of life
  • Provides a credible role model as a healer and human being to his/her community, and as a professional in the science and art of medicine to colleagues, other health professionals, to young physicians in training and medical students

Students and residents are encouraged to send letters of nomination to the Wisconsin Academy describing why their nominee should receive the Family Medicine Educator of the Year Award. This allows students and residents to recognize the excellence of their teachers.

To nominate an individual for this award, please provide the following information:

  • Nominee's name, address, and business phone
  • Award for which the individual is being nominated
  • A few paragraphs describing why you believe the individual is a good candidate
  • Any supporting material such as newspaper articles, etc., (optional)
