The NCQA has established a process for certifying practices as Patient Centered Medical Homes. We would like to know whether or not your practices are interested in such certification and where you might be in that process.
We are not interested and will not be pursuing PCMH certification
Not on our radar screen at this time but we may be interested in the future
We have discussed seeking certification some time in the future
We have started activities to prepare for PCMH certification
We have purchased the NCQA PCMH module
We have applied for certification
