This two-part webinar series, Achieving the Triple Aim with Team-Based Care, was developed to provide family physicians with a team-based care approach to treating Chronic Kidney Disease.  This series was supported by an unrestricted grant from the National Kidney Foundation of Wisconsin through the WAFP-Foundation.

This activity has been reviewed and is acceptable for up to 2 Prescribed credits by the American Academy of Family Physicians (AAFP). AAFP certification begins March 5, 2014. Term of approval is for one year from this date with the option of yearly renewal. Physicians should only claim credit commensurate with the extent of their participation in the activity.
